Consent of Instructor Permits for Undergraduates

College and Registrar Forms
What classes require Consent of Instructor?

All graduate level (6000 level) classes fall into this category and many 5000 level courses do as well. Sometimes there are prerequisites specified in the flyer that has been published for the course, and sometimes it is up to the professor to determine if you have the background needed to be sucessful in the course. Some 4000 level courses such as selected topics courses also may require consent of instructor in addition to prerequisites.

N.B These instructions are for undergraduate students.

Bring your SASS or unofficial transcript from FACTS.org to a pre-arranged meeting with the professor. Bring the Consent of Instructor form as well.

When the form is signed, the professor will take it to the office so a permit can be processed. This may happen during your meeting or later depending on the professor's schedule. Check your OASIS Registration Status to see when the permit has been granted.
