Department of Biology Emergency Procedures
If the emergency requires medical attention, police assistance, or fire, then
call 911.
If you are injured on the job refer to University of South Florida's Environmental
Health and Safety's Workers' Compensation web site (see Useful Links). Several forms
on the Workers' Compensation Web Site will need to be filled out within one working
day of the incident.
Individuals participating in research activities who are not students and not employees
need to be treated as volunteers and have the appropriate documentation. Use the
link to the CAS Office of Facilities, Safety and Security for these forms and instructions.
Additionally, you must complete a
Biology Incident Report Form (PDF), and submit the form to Bruce Smith or
JP Swigart (mailboxes are in SCA 110).
